What Are Spinal Cord Injuries?

According to the National Institute of Neurological Disorders and Stroke (NINDS), the definition for a spinal cord injury is as follows: “A spinal cord injury usually begins with a sudden, traumatic blow to the spine that fractures or dislocates vertebrae. The damage begins at the moment of injury when displaced bone fragments, disc material, or ligaments bruise or tear into spinal cord tissue.”

Although most spinal cord injuries do not sever the spinal cord, fractures and compression of the vertebrae can destroy axons (nerve cells), which disrupts the communication between your brain and the rest of your body. In severe cases, this disruption can lead to paralysis.

Incomplete vs Complete Spinal Cord Injuries

All spinal cord injuries are either incomplete or complete. An incomplete spinal cord injury allows some function below the primary level of injury, but a complete spinal cord injury produces the total loss of all motor and sensory function below the level of injuries. The American Spinal Injury Association (ASIA) breaks down spinal cord injuries even further with the ASIA Impairment Scale. The ASIA “grades” are as follows:

  • ASIA A is a complete spinal cord injury with no sensory or motor function
  • ASIA B is a sensory incomplete injury with complete motor function loss
  • ASIA C is a motor incomplete injury where less than half the muscles can lift up against the force of gravity with a full range of motion
  • ASIA D is a motor incomplete injury where more than half the muscles are “anti-gravity” (see above)
  • ASIA E indicates a full recovery

Spinal cord injuries can also be classified by where the injury takes place or the level of injury. Generally, the higher in the spinal cord the injury occurs, the more serious the impairment. People are also less likely to recover from more severe injuries. Cervical spinal cord injuries, for example, happen in the 7 vertebrae in the neck and frequently result in quadriplegia or tetraplegia (paralysis from the neck down).

Similarly, thoracic spinal cord injuries affect the chest and legs, resulting in paraplegia (paralysis from the waist down). Lumbar and sacral spinal cord injuries usually result in some loss of function in the hips and legs.
